The way best way to reach my office is by calling 734-984-2228. If you reach my voice mail, please leave your full name, age, phone number and a brief description of their current concerns. I will personally review the information and call back within 3-4 business days for a short phone consultation and then discuss with you about setting up an appointment. Fees and insurance

The psychiatric services provided at this location, is based on a fee-for-service model. Having minimal focus on the administrative problems allows Ashwini Gulwadi, PLLC practice to provide more comprehensive patient-based treatment model, which is completely based on patient needs. Payment is expected at the time of each visit via credit card or cash or check payable to “Ashwini Gulwadi, PLLC”. The patient is responsible for all costs at the time of the visit. Fees are based on the length and complexity of the appointments and based on individual needs; and can range from $280-$480 for initial evaluation and $150-$240 for follow-up appointments. This course of action makes us unable to file a claim to insurance companies but maximizes your privacy and puts you in control of what information you make available outside of this office.

If you have an insurance plan, the services provided here will be considered out of network services. We can provide you with a statement or services if you choose to submit to your insurance company for reimbursement. If any additional information is needed, we will work alongside you to provide what is needed to process your claim. Unfortunately, we cannot guarantee that your insurance company will reimburse you, since insurance policies vary so widely with respect to how psychiatric services are covered. When calling your insurance for benefit and coverage questions, it is imperative that you tell them it is for Psychiatric services, as most insurance companies handle these claims differently. We always recommend speaking with a customer service representative with your carrier, to get the most current, accurate information available. Questions to Ask Your Insurance Provider: Do I have Mental Health benefits? What are my Out of Network benefits? Is there a deductible before I am reimbursed? Can I submit my own claims? What is your reimbursement rate? Do I have a maximum amount of money or visits that are allowed? How can I use my FSA or HSA account and submit the claims to the insurance company?

For changing appointments/cancellations

At least 72 business hours’ or 3 business days’ notice is required in order to allow for changes in my schedule to accommodate all my patients. For all late cancellations and no-shows (including new patient assessments), you will be charged full cost of the scheduled session, which is due before you schedule any future visits or new patient consultations.

In terms of intake paperwork and forms, they need to be filled out prior to your initial appointment. Completing this paperwork prior to the visit will allow us to use your session time to its fullest. If you are doing in person visit, the forms will be available in a bin outside my office door. Please allow 15-30 minutes prior to your appointment to complete these forms in the waiting room. If you are doing tele visit, the forms with be emailed to you and they need to be completed and emailed or faxed or mailed to my office prior to your scheduled appointment date.

Tele-Health Appointment Information

  • Intake forms will be emailed to you before your first appointment. Please fill them out and return them before your first appointment.
  • You will need a computer or android or apple devide to connect to the video call platform.
  • You will also need a valid email address where the link for tele visit appointments will be sent.
  • Payments may be made with credit card, debit card, or bank transfer via an online invoice. You will be getting an invoice via your email, with a link to use your credit card for payments.
  • Payment are due on the same day as the day of services. All dues must be paid in full before your next appointment in order to continue treatment as planned at Ashwini Gulwadi PLLC.
  • If you need statements for services provided, please let us know. This can be sent by mail or as an email.
